Social Care Networks (SCN)


Helping Medicaid Managed Care members meet essential needs like housing, food, transportation, and more.

MHA in Orange County, partner of New York’s Social Care Network is here to connect individuals to non-medical supports that promote long-term wellness. Services begin with a Social Determinants of Health screening that helps identify Health-Related Social Needs (HRSNs).

Eligibility
To qualify, individuals must:
• Have active Medicaid through a Managed Care Plan (e.g., Fidelis, MVP, Healthfirst)
• Live in Orange, Ulster, Sullivan, Dutchess, Putnam, Rockland, or Westchester County
• Complete a brief screening for needs like food insecurity or housing instability

Available Supports
• Rent or utility assistance
• SNAP or Medicaid help
• Medically tailored meals
• Transportation for critical needs
• Housing search and tenancy support
• Referrals to local and state resources

Participation is voluntary and starts with a conversation.
